Authorization for Release of Medical Information

How to Obtain Medical Records
Medical records for patients of The Hospice of Moorestown VNA are maintained by Moorestown Visiting Nurse Association. A patient or the patient's legally authorized representative may request a copy of his/her/their medical records. This request must be in writing and include proof of Power of Attorney (POA) if applicable. If preferred, a Medical Record Request Form can be sent to the individual or his/her/their representative. Request letters or completed forms should be mailed to Moorestown VNA, Attn: Medical Records Department at 300 Harper Drive, Moorestown, NJ 08057. A legible photocopy of the record shall be sent to the requester within 2 business days.
